Issue with Benelli M4.

Cycle the bolt a few times. Bolt is now in battery. Insert snap cap into magazine. Depress cartridge release lever. Sometimes the shell is moved into the magazine loading port. Then I cycle the bolt and the shell inserts into the chamber.

But, sometimes, after cycling the bolt, the bolt is in battery. I insert shell into mag and depress cartridge release lever. This time, the shell remains in the mag. I pull the bolt back and instead of loading a round, all that happens is that the bolt simply locks open (with snap cap still in the mag).

I can follow the same procedure but get two different results.

What am I missing?
