Not mine. I wish! All I know is that it's the older Ithaca Model 37...before they began making them again. Pulled the photo from a download I kept when I was researching the Ithaca 37s. You guys probably know much more about 'em than I do.
But ya, you both pinned down the make/model pretty darned fast.
