Always +1. I read recently that not having a round in the chamber can cost as much as two seconds to present the firearm and chamber a round. If you ever get in a situation that you actually need to present your firearm, you would probably have a better survival chance, unless your opponent is a miserable shot, if a round was chambered and all you had to do was click down on the safety.

I carry with one in the chamber, but I don't top off my magazine. I do usually carry a second magazine.
I don't particularly enjoy messing about with my gun after it's hot, and I'm not convinced 31 rounds is going to get me out of a mess that 30 won't. EDIT: with my BG380 it is noticeably harder to insert a full mag into the gun with the slide closed. Loading is as such seems to also make the mag release far more touchy.

A while back I read something from Chuck Taylor about under-loading high capacity mags to keep the springs from wearing down as quickly. I don't under-load them, but I don't top them off. I load a full mag and chamber a round and leave it at that.

Depends on the gun and magazine. Some of my mags top off and go back in the pistol smooth, as a rule, if I have to force the mag back in, I usually take one round out. I do not like feeling like I am forcing it back in. YMMV.
