Questions about IN laws

I have a couple questions regarding carrying in Indiana. I have searched high and low and not gotten the exact answers im looking for. I need/want a list of the exact places one can or cannot carry a concealed weapon in the state of IN. I have done a rediculous amount of searching and found vague references but I want something that abruptly says I cannot carry in X place, Y place, Z place, etc.

2nd, after receiving my new CCW permit, i recently moved. Who do i need to write to have my address changed. I know I need to inform the ISP in wiritng of my move, but i dont know who to put the attention to or where to write it to.

Normally you would notify the issuing agency.

if they have a website you can download print and fill out the form from there.

It is called something like an address change form or something to that affect.

for example DPS (Department of Public Safty) is our issuing Agency and i would send the Address Change form to them with the new address on it

Got the form for the adress change. it wasnt called address change, but close enough for me to figure it out.

Now all i need is the specifics of where i can/cant carry.
