Out of curiosity, click on the space just below the word 'extensions' in that block and see what happens. Maybe the button graphic is missing, but the button properties are present.
Do you see the other control elements in the Additional Options window? There should be 4 check boxes and 2 selection controls in addition to the missing Attachments button. At the bottom are repeats of the Submit Reply and Preview Post buttons.
