That is measuring outside edge to outside edge, then subtracting the diameter. It wasn't said and no doubt somebody would get it wrong.
I can shoot 8" groups with my REM 870 (18" barrel) at 100 yards, but I don't think the lack of accuracy is the gun, but the shooter and sights. It is a little tough placing a single front bead sight on the exact same spot each time when the bead is visually huge compared to the target at 100 yards.
