Yes, the shotgun is an added modification.

It appears to have started out life as a Belgian Model 1889 standard infantry rifle, which would have had a straight bolt handle.

At some point in the past, and likely done by one of the hundreds of gunshops located in Belgium, it was heavily modified to add the dual trigger, turn the bolt handle down (and VERY poorly reshaped by grinding), mount the scope, and add the shotgun.

The work is FAR too crude for it to have been done by FN.

Belgium was literally dripping in gun shops turning out a wide variety of firearms and modifications. My take, this was done by one of the lesser shops.

It's interesting, but it's quite the bodge job.
