buddy of mine has ben waiting for almost 2 months now and he called the local office and they said they are behind with all the new requests (the head office is behind that is not local office) . He got lucky though because the local office in phoenix sent him some forms they had on stock and it took only 2 days.
when you do order forms make sure to order at least 2 so you have a spare in case you mess up the other form.
