Forgive the heresy, but an inexpensive .45 acp that satisfies most of your requirements, particularly reliability with all ammo, is a Ruger Blackhawk convertible. Fairly lightweight, fits small hands, laughs at odd-assorted ammo, reliable as a hammer. Generally, quite accurate. Fails miserably when it comes to a quick reload, though.
