The problem appears to have been with the bindery at Brown Printing. Several hundred copies were affected in the early portion of the print run. These issues show missing and duplicate pages between page 21 and 29.

We're set to replace defective copies immediately. And we've decided to offer a TWO MONTH EXTENSION to everyone who notifies us of a defective copy via the link below by Friday August 30. After that date, we'll still be offering Replacement Plus One Free Issue.
