Six-shot?
The serial is OK, but isn't necessarily the final determinant of the exact model.
The other numbers are assembly numbers, if it's a Smith.

With the cylinder open, do you see a Mod and a number on the frame just below the barrel?

If it's a four-inch, six-shot, adjustable-sighted revolver you've probably got a K-frame Smith. Does it say Smith & Wesson on it? Other makers had CTG as part of their caliber stampings on the barrel.
