Inspect the involved magazine. Check to see if the lips are straight and unbent. Also check to see if they are tapered the same degree. To make it easy, compare them to an unused Colt or GI magazine.

If your magazine is faulty, set it aside and never use it for emergencies/self defense. It's only good for training (malfunction drills)
