My copy has pgs 21-24 and pgs 57-60 inserted twice, while pgs 25-28 and pgs 53-56 are missing. I'll be filling out the form, and returning the magazine as requested, Rich.
For those who are also affected by this, Here's how to find your subscriber number:
At the top of your address label is a line of asterisks. Directly beneath that line is a ten digit number between two pound signs. (#) This ten digit number is your subsciber number.
