The human heart does not sit under 12" of tissue.
I think 12" is a generalization.
12" of ballistic gel is the accepted standard.

The human chest is considerably tougher than gel - ribs, cartilage, connective tissue, etc. .... 12" of gel is considered adequate penetration.
