I don't know that it's of any help to you, but I own a H&R Bay State Arms .410 single with a 4-digit serial number, that was made ca. 1917.
It has a nickled receiver with a blued barrel that has the same "CHOKE" marking atop it:

Welcome to TFL.

From the serial number posted, your shotgun is either a #5 or a #8, manfactured in 1940.

I have an old H&R baystate 20 ga. with a serial number of 19108. Ive exhausted myself trying to find info on manufacture date. The serial number beside the trigger guard is a19108. It also says made in u.s.a. 20ga choke on the top of the barrel. Can someone please help. Thanks.
