Otis cleaning questions

My parents got me an Otis cleaning kit for Christmas.
It is not bad and covers everything I have and then some.

But I had issues trying to get it to work with a shotgun (12ga).

I found the little rubber disk you put on the brass piece which you attach the patch to just come off and then I had to get out my cleaning rod to poke it out...

Any tips?

You aren't putting the patch on correctly. did it come with a DVD? I think mine did and the DVD explained how to do it. You can probably find the video on their website or youtube also.

Mine did not, it only have with 2 or 3 pictures which were not very good. After looking around more I think it was me not looping the patch back through the loop properly.
