When did this form first come into existence? I'm sure my grandfather or father never would have had to fill out one of these things.

Not sure of the year (possibly 1968) but I can never remember NOT having to fill one out.

I believe it was done as part of the gun control act of 1968.

I know it was in existence prior to the NICS because the brady's used the phrase "lie then buy" to help in getting the brady bill passed.
