C&R address change

I moved a while ago, and I printed off an "application for an amended federal firearms license". Its a C&R license, so it obviously isn't for a business. I just filled out my old mailing and home address and my new address, along with my license number. Is that all I need to do? The rest of the stuff seems to only apply to businesses. Also, where do I mail this? My regional office or the big one in Atlanta?

I did this a year ago. I just wrote "collector/FFL03" whenever it asked about my "business." Mailed it to the main office...same place as the original. They will send a new license with new address and license number, but the old expiration date.
