Looking for training in New York.

I have my CCW in Western New York. I am looking for training in personal defense and or survival.

I have a Medical condition that makes it very painful to walk any distance. I am looking for one on one training due to my condition. In addition I have been thinking of taking survival training courses I would think they would have to be tailored to a person who can not walk very far or for god sake run!

Please let me know if you have any suggestions.
