"Certificate of Compliance U.S.C 18" Need Help asap...

Hello...
I was unaware of this form until tonight and am signing paperwork tomorrow...To late to call ATF and ask, so need help with a question...

On Line 1, it asks "Name under which application was filed"...
Does this ask for the Transferor or the Transferee???

The transferor does not know either and tomorrow is SAT, so I'm stuck...Anyone who knows, please advise...

The way I was instructed by my FFL to complete it was the following:

1. Put your name in the first set of boxes.
2. Put your name in the second set of boxes.
3. Put USA in the third set of boxes.
4. Sign and date on line 7.

This is all assuming you're doing a sale under your own name, and you're a citizen.

Yes, you're signing a form swearing that you're a citizen. Because you know, Al Quaeda always attempts to get rare, expensive class 3 weapons the legal way, instead of smuggling in $100 AK's. This form will prevent that, because they wouldn't lie, would they?
