Fill out a form (about as simple as a 1040-EZ) in duplicate, send one copy to the BATF along with a check, or similar, send the other copy to the chief law enforcement officer in your area (sometimes city, sometime county...varys by area), wait about 3 months. If you filled it out correctly you'll receive your license.

Go to the ATF website and request an application packet via email, it will arrive in a week or two, fill it out, make a copy, sign, send.

No

Edited: You do not need to provide fingerprints for C&R license.

The check you need to send is for $30 (that was the cost with my last renewal). It's the best deal going!

I don't believe Makarov pistols are C&R eligible. If you want something else that shoots 9mm makarov and is C&R, look at the cz82. Awesome gun for ~$200.
