OK, someone screwed up and it's not the machinegun. Now I can see why the guy behind it raised his hands. I'm wondering how much out battery/chamber the round was when it went off? Good thing wall-to-wall counseling is not enforced in the field.

Isn't that a Quick Change barrel? Just not correctly installed maybe?

NOt installed correctly.

Plus, you can hear the audio "Didn't tighten the barrel back up, did you?" "no".

I saw photos of a cut-down M2 somewhere, but I could not find them when I looked just now. The muzzle-flash was obscene.

YEP my TC launched his M85 50cal after not locking it in all the way. Happens more often then you think.
