Loose Bullets

All, I'm sort of new to reloading. I need help understanding why some of my bullets are loose after the final stage of reloading?. I shoot competative HP and haven't seen any real difference lately in my scores. Here's what I'm using

223
Lake City Brass, once fired
69 G. moly and some naked sierra MK's
RCSB Pro2000 Progessive
RCBS Small base dies
Horn case lube spray

So, tell us!

I had this happen with a oversize RCBS die. I tried crimping the snot out of the rounds and then used a Lee die on the neck and that was all it took.
