Para LDA 7.45 Pops Safety Out...

My buddies Para LDA keeps popping the safety completely out of the gun while firing and dings the slide where it engages. He's a lefty and has a ambi safety installed. I suggested that he may be letting his thumb pop it up under recoil but he insists that he's positive that he's not allowing that to happen.

It's been back to PO once on this and still does it. Any ideas on what could be going on internally that would allow this to happen?
