I am SBRing an MKE AT94k (Sp89 clone). I searched the forum and I couldn't find the answer.

On the Form 1, do I list myself as the original manufacturer since I am creating the NFA weapon or do I list MKE as the original manufacturer?

Also, do I need to send in two sets of fingerprint cards? My police department charges $20 per set, and I want to make sure I bring enough FBI cards and cash.
