It's a "Victory" model made during WWII.
SN's ran from V1 in 1942 to V769000 in 1944.
What distinguishes yours from regular US "Victory" models is the Cogswell & Harrison marking. S&W sold quite a few of them to the British as the model .38/200 (K-200). Those where chambered in 38 S&W.
Following the war a lot of the guns were reimported back to the US and C&H would convert them from 38 S&W to 38 Special.
