Have a gunsmith check the headspace. This is fairly common in older lever action rifles that have the locking lugs at the rear of the bolt.
Excessive headspace means the case head is not rigidly supported when fired. This allows primer setback at first, then possibly separated cases.
If a new bolt can be obtained that might correct the problem, otherwise the barrel may nee to be set back.
Check with your gunsmith.
