Louis L'amour in several of his stories mentions has several caracters carry a derringer up their sleeve. I think the way this is done for a right handed person is to put the gun up the left sleeve using an elastic band to hold it to the wrist. The derringer may then be drawn with the right hand. Bringing ones hands together is not a motion which advertises that one is reaching for a gun. This enables a fast unexpected draw.
It is really a very simple trick and as such not dramatic enough for a movie, hence all the complex gadgets on which only an actor can afford to "risk his life." In real life keep it simple, its more effective and safer.
