One possibility that I've had to deal with is grit getting inside the bolt and slowing it down so that there is no primer ignition.

I check this with a piece of dowel (long enough to have weight). I ensure the firearm is empty, put in the wood dowel, point the firearm straight up and pull the trigger. I'm looking to see how forceful the firing pin hits the wood dowel.

This is also a good method for lowering the firing pins on shotguns... especially fine doubles.
