I was duck hunting with a guy that had a model 12. A pair of black duck went over the flooded timber we were in and he fired once and ejected the empty and pumped a new shell in and the gun went off. He stopped and looked at the gun and said to me did I see that and he said he must of had his finger on the trigger when he slammed the action shut. It was long and he bought a new 870 and I think it was because he did not feel the Model 12 was safe.
