1. On Line 2 if its a trust, do I check Individual or Corporation?
2. On line 3b for Applicants name and address, do I use my name or the trust name?
3. On line 4a do I need to furnish plans and specs?
4. Does a trust need to fill out a form 5330.20 to certify citizenship?
